It's that time of year again to consider increasing your life insurance through SEBA's MetLife Insurance program. Although all members have $2,500 basic life coverage by SEBA, now is the time to increase your coverage up to $500,000!

During this open enrollment period, you may elect to begin or increase your life coverage up to $250,000 and your spouse's coverage up to $50,000 – both without medical exams. Or you may choose more coverage, up to $500,000 for you and up to $250,000 for your spouse.* Your spouse may elect up to one-half of your coverage.

In addition, you may choose Voluntary Accidental Death & Dismemberment (AD&D) coverage up to $500,000 for yourself, or choose the Family Protection Plan to cover yourself and your family.

The MetLife plan also includes a number of other Special Features, such as Travel Assistance and Will Preparation Service.

If you choose to begin or increase your coverage UP TO $250,000 for you and/or UP TO $50,000 for your spouse, complete the Enrollment Form for Group Insurance.

If you are requesting OVER $250,000 life coverage for you and/or OVER $50,000 life coverage for your spouse, complete BOTH the Enrollment Form for Group Insurance AND the Medical Evidence of Insurability.

Return all forms to SEBA, 735 E. Carnegie Dr., Ste. 125, San Bernardino, CA 92408. DO NOT MAIL TO METLIFE or your coverage may be delayed.

If we receive your enrollment information postmarked by September 17, 2010, your coverage will become effective on November 1, 2010. While enrollments are welcome following the effective date, they will be subject to medical evidence of insurability.
