American Specialty Health (ASH) Chiropractic Benefit

SEBA is pleased to provide its members with exception chiropractic care from American Specialty Health Plans of CA (ASH Plans of CA). ASH Plans of CA was the nation’s first chiropractic HMO and currently contracts with California’s largest health plans.
To access care from one of ASH Plans of CA’s participating providers, just follow these three simple steps:

Select an ASH Plans of CA contracted provider of your choice.
- No referral required from your Primary Care Physician.
- You may change providers at any time.
- Call the ASH Plans of CA contracted provider of your choice to schedule an appointment.

Bring your ASH Plans of CA Identification card, and when you arrive for your appointment the following will occur:
- The provider will contact ASH Plans of CA to verify eligibility status.
- You may be asked to complete a health history form.
